Can you use clindamycin with benzoyl peroxide?
One of the few skincare pairings where the answer is not just yes, but please do.
Short answer: Yes. Topical clindamycin and benzoyl peroxide are a recommended combination, because benzoyl peroxide helps prevent the bacteria from becoming resistant to the antibiotic. Many prescriptions combine them in one tube. What deserves more care is what else you layer with clindamycin: exfoliating acids add irritation, and a retinoid, if prescribed alongside it, should be used exactly on your prescriber's schedule.
Why this pairing is encouraged
Used alone for months, a topical antibiotic gives acne bacteria time to adapt. Benzoyl peroxide works differently, by oxidation rather than as an antibiotic, so bacteria do not build resistance to it. Using the two together keeps the antibiotic effective for longer. Dermatology guidelines specifically recommend pairing topical antibiotics with benzoyl peroxide rather than using the antibiotic on its own.
How to use them together
- If you have a combination product, use it as directed and you are done.
- If they are separate, the usual approach is benzoyl peroxide in the morning (or as a short-contact wash) and clindamycin at night, or both applied a few minutes apart. Follow your prescriber's directions over any general advice.
- Benzoyl peroxide bleaches fabric. Use white pillowcases and towels.
What to be more careful with
- Exfoliating acids. Salicylic, glycolic, and similar acids raise irritation on skin already being treated with clindamycin. Check with your dermatologist before adding one.
- A retinoid. Clindamycin and a retinoid are commonly prescribed together, and the retinoid adds dryness. Use both exactly as prescribed rather than adjusting the schedule yourself; your prescriber may build the retinoid up slowly.
- Isotretinoin. Topical antibiotics are usually unnecessary during an oral isotretinoin course. Ask whether to keep using it.
Erythromycin follows the same rules
Erythromycin gel is the other common topical antibiotic for acne, and everything above applies to it too: pair it with benzoyl peroxide, be careful with acids, and use a retinoid alongside it only as prescribed.
